Decision log vs. action tracker: stop losing the why and the next step

Keep decisions and actions separate but linked. A simple decision log and action tracker help teams see the reason, owner and next step.

"We agreed on this" and "someone is doing this" are different facts. Keep both visible.

Atlassian's DACI framework separates the person driving a decision from the one person who makes it, the contributors and the people who need to be informed. Its guidance also asks teams to record the final decision and the context. You can use that distinction without buying a new tool.

The decision log records the call

Use a short record: question, decision maker, date, options considered, evidence, decision and reason. Add what would cause you to revisit it. Link the source material instead of copying every conversation into the log.

Do not label an idea as a decision because it appeared in meeting notes. If approval is still needed, write "pending" and name who makes the call.

The action tracker records the work

Use: action, owner, due date, status, definition of done and blocker. Link it to the decision that created it. One decision may create several actions; an action may also reveal that the decision needs to be revisited.

Atlassian calls the decision roles Driver, Approver, Contributors and Informed. Driver does not mean final authority. Keep that clear when a Chief of Staff is moving the process along.

A fictional example

Decision: test a new onboarding checklist for the next intake, rather than replace the whole system. Reason: learn where the current handoff fails before committing to a larger change.

Actions: an owner drafts the checklist; a reviewer checks it; the team records what happened during the test. Give each action a date and a clear completion condition. These are invented teaching examples, not a client result.

Review the exceptions

At your next check-in, focus on overdue actions, unresolved decisions and new evidence that changes an earlier call. If something is blocked, ask whether the missing piece is effort, information or authority.

A tracker should help someone act. A log should help someone understand. If the same meeting keeps happening, check which of those two records is missing.
